Please note, all appointment changes and cancellation require 24 hour advance notice by phone. There will be a $50.00 late fee charge for any changes made without advance notice.

Drug and Alcohol Evaluations/Assessments

Often when an individual is charged with a DUI (or any other drug or alcohol related charge), he or she will be required to undergo an evaluation either by a judge, attorney or probation officer.

Anger Management Evaluations/Assessments

A judge, attorney or probation officer will often require an individual who has been charged with domestic violence, simple battery or assault to also undergo an anger management evaluation.

What To Expect

If you are coming in for a court ordered evaluation you will fill out a few forms and a questionnaire. Then we will talk about your situation (this will take about an hour). Approximately one week or less after our meeting, provided I have all the information I need, I will send a typed report to you and your referral source such as a probation officer or an attorney.

Fees

Fees for court ordered evaluations range from $150-$200.
When possible, expedited reports are available for an additional fee: $50 for reports completed before the end of the next business day; $30 for reports completed within 5 business days.
Payment is expected at the time services are rendered and I accept payment by cash, all major credit cards and debit cards (no personal checks please).
Please note that additional charges will apply to extra paperwork required for some out of state evaluations.

Phone Evaluations/Assessments

The evaluations listed above are also available via phone appointments for $150 (the expedited fees listed above also apply to these evaluations). We encourage you to check with your probation officer or attorney first to verify that this will be acceptable.

When you contact our office please indicate that you are requesting a phone evaluation. We will need to collect payment upfront to e-mail or mail to you the forms to complete. You can use a credit card or you can mail a cashiers check or money order to our address (no personal checks please). Once you’ve completed the forms you will send them back to us. When we receive your forms we will contact you for the phone interview.

Please note that we cannot provide DHR Approved Clinical Evaluations via the phone, these have to be face to face.
