• If you have been convicted of your first DUI and your arrest date was on or after 7/1/2008.
   
• If you have been convicted of more than one DUI within a 5 year period, and your first arrest date was before 7/1/2008.
   
• If you have been convicted of more than one DUI within a 10 year period, and your first arrest date was on or after 7/1/2008.

If you require a DHR Approved Clinical Evaluation for either a first DUI conviction (if your arrest was on or after 7/1/2008) or multiple DUI convictions, you will first have to complete the DUI School and have them fax to me the following information (we can not schedule an appointment until I have these forms):

• Your NEEDS Assessment results from the DUI School (approximately 6-7 pages). Please note that you can not bring this information with you, it has to be faxed directly to me from the DUI School. You will have to go to the DUI School and sign a release.
   
• Paperwork from the DUI Schools can be faxed to 770-953-0031. My DHR Clinical Evaluator number is 1642. Please call me to make an appointment once your paperwork has been faxed.

Please bring with you the following information:

• Your letter of completion (from the DUI School).
   
• A copy of your ten (10) year driving history/record (or your 7 year driving history/record, if the 10 year is not available). You can get this from the DMV. You can go to www.dds.ga.gov.

We can then schedule an appointment to meet for approximately 1-2 hours to complete the evaluation.

Note: Please let me know if you have completed any substance abuse counseling prior to your evaluation.

For more information about the new changes to the DUI laws as of 7/1/2008, please go to the Georgia General Assembly website at www.legis.ga.gov. To view House Bill 336, follow the directions below:

1. Click Legislation on the top of the page.
2. On the left side of the page, under Legislation click Search.
3. Fill in the following fields:
   -For Session, choose 2007-2008 Regular Session
   -For Bill Type, choose House Bill
   -For Number, type 336
4. Click Search. House Bill 336 will appear below the search fields.
5. Click HB 336 to view the bill.

• Fees for DHR Approved Clinical Evaluations range from $150-$200.
   
• Payment is expected at the time services are rendered and I accept payment by cash, all major credit cards and debit cards (no personal checks please).
